Medication Guidelines

Livingston Parish Public schools require the following forms to be on file BEFORE any medication can be administered at school: 

1) Signed authorization form permitting medication to be administered at school.  Must be signed by a parent or legal guardian.

2) Signed medication order.  Must be completed by the child's licensed prescriber.  Must be renewed if there are any changes to dosage, time, or medication itself.  Additionally, this form must be renewed at the beginning of each school year.

Procedures

  • Medications should be delivered to the school office by a parent or other responsible adult.  Students may NOT deliver their own medication to the office.  

  • Medication must be in a container with a label from the pharmacy.  If necessary, your pharmacy can provide separate bottles for school and home use.  

  • A thirty-five (35) day supply is the maximum amount of medication to be delivered to the school at a time.

  • If the prescription requires that a tablet be cut in half, it is required that the parent break the tablet.

  • Parents must pick up empty container.  Students cannot bring containers home.  

  • Over-the-counter medications to be administered at school will need a doctor's order and prescription label.  

  • Any time a student is prescribed a new medication, or dosage changes  for current medication, he or she must remain home for 12 hours after the first dose to observe for any adverse reactions.
